The NHS has admitted \u2018shortcomings\u2019 after a hip hop star died after falling out of a third-storey window.<\/p>\n

In April 2020, Foreign Beggars\u2019 musician Ebow Graham<\/a> \u2013 who performed under the name Metropolis \u2013 died at the age of 41.<\/p>\n

His death was confirmed by the UK hip hop group on their Facebook<\/a> page, who announced he\u2019d died following an \u2018accident\u2019 at his home.<\/p>\n

At the time they wrote that Graham, the father to a 10-year-old-son, was a \u2018deep and peaceful soul, a dedicated and loving father\u2019 who also \u2018touched people with his warmth, insight, talent and generosity everywhere he went\u2019.<\/p>\n

Now, five years on, the East London NHS trust and London ambulance service have admitted failures that led to his death in a damning report.<\/p>\n

Following a psychotic episode, Graham fell 40 feet out of a window from his flat in Clapton, east London.<\/p>\n

It\u2019s now emerged that hours earlier, London ambulance paramedics had dismissed Graham\u2019s friends\u2019 concerns about his behaviour, while a crisis call handler from the trust also failed to properly assess his risk or arrange an urgent mental health assessment within the recommended time.<\/p>\n

Both have now reached an undisclosed settlement with Graham\u2019s former partner Francesca Macri.<\/p>\n

As reported by The Guardian<\/a>, his friends first called 999 around 4.20pm on April 17 when they became worried for him.<\/p>\n

Macri has said that when paramedics arrived, they were \u2018dismissive and flippant\u2019 and concluded the musician had \u2018capacity over his care\u2019.<\/p>\n

A serious incident report that wad filed by the London ambulance service later admitted that they had failed to follow appropriate protocols by not contacting the mental health team for advice.<\/p>\n

Two hours later at 6.30pm his flatmate, a GP, called the mental health crisis team urging for assistance as there were still grave concerns about Graham\u2019s state of mind.<\/p>\n
